Facility Evaluation Report

On 09/11/2025, Licensing Program Analyst (LPA) V Gorban conducted an unannounced Case Management visit. LPA introduced self, stated purpose of visit and allowed entrance. LPA met with Administrator Assistant Monique Balladares and both conducted the visit.

The purpose of this visit is regarding the incident report recorded on August 23rd, 2025 client (C1) observed smoking a pipe. After the incident, Licensing office and client's conservator were notified. Facility conducted a meeting regarding the incident and with clinical team, case worker, conservator and facility staff. Based on interviews, C1 was offered recovery program and observation the next 30 days. Client was notified with warning of potential consequences if house rules not followed.

There were no deficiencies cited at this time.

An exit interview was conducted and a copy of this report provided to administrator assistant for facility records.
