Facility Evaluation Report

On 09/11/2025, Licensing Program Analyst (LPA) V Gorban arrived unannounced for an Annual Required Inspection. LPA met with Administrator assistant Monique Balladares. LPA stated the purpose of the visit and toured the facility conducting health and safety checks. Facility has one main entrance/exit point.

Facility appeared clean with no obstruction or fire clearance issues. Hand sanitizer was readily available to residents and visitors. Social distancing is maintained in the common and dining areas. Bathrooms have trash cans with lid. Hand washing posters were observed by the bathrooms. Bedrooms were checked. The exterior tour was conducted.

LPA checked residents’ medications room. Facility extinguisher service date recorded on 10/04/2024. Last recorded disaster drill conducted on August 12th, 2025. Clients observed during lunch time in common area. Dinning room and kitchen were toured. Food supply was checked and there appeared to be an adequate supply. Refrigerator temperature observed at 30 degrees F, and freezer observed recorded at 1 degrees F. Cleaning and PPE supplies were checked in storage outside and locked.

Staff records were reviewed for good health and infection control training. Facility staff was observed being present and available to clients. Client’s files reviewed with updated required information.

No deficiencies observed or cited during this visit.

Exit interview was conducted, report signed and provided for facility records.
