Facility Evaluation Report
On 02/04/2025, Licensing Program Analyst (LPA) V. Gorban conducted unannounced visit to conduct case management, health and safety checks visit on clients in care. LPA introduced self and allowed entry in to the facility by care giver staff Percy Von Winning. The facility operator Trevor Grant was notified of Licensing visit.
During this visit LPA toured the facility and interviewed operator, staff and clients. During the visit all three clients were present getting ready for dinner. LPA reviewed staffing schedule and other facility records and interviewed operator. Based on operator interview C1 is waiting on funding and ready to move to stay with her family. C2 has an appointment with regional center next week to update IPP with a plan to relocate C2 with Trevor Grant to his Licensed facility. C3 has interview next week over zoom with one of the San Jose facilities administrator.
No deficiency was observed and cited during this visit.
