On 11/8/24 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management. LPA met with Trevor Grant.
LPA requested current/recent LIC308, LIC200 (if there is a more current), and verification of control of property. Trevor will provide current verification today. Trevor explained other documents can be provided once he is able to contact his attorney. The attorney is not available until after 11/13/2024.
Exit interview was conducted and a copy of this report was provided to Trevor Grant.
