Facility Evaluation Report

On 10/23/25, Licensing Program Analyst (LPA) V Gorban arrived unannounced to conduct a case management visit regarding the incident report. LPA introduced self, stated the purpose of the visit and met with Resident Service Director Nina Francetic. Administrator Chris Regan was notified and was able to attend the visit.

The purpose of the visit is to follow up on the incident reported to Licensing office on 09/16/25. Per report, R1, when returned to her room, was not able to locate her personal items that were left in the room with the front door left unlocked. Licensing office was notified via incident report. Law enforcement was contacted and sheriff visited the facility on 09/12/25 to conduct the investigation. Per facility no event number provided.

LPA requested facility records for review and will follow up with the facility at later time.

No deficiency was observed or cited during this visit.

An exit interview was conducted. A copy of this report was provided to administrator, whose signature confirms receipt of this report.
