The inspector’s account

On 06/27/2025, Licensing Program Analyst (LPA) V Gorban conducted an unannounced Case Management visit. LPA introduced self, stated purpose of visit and allowed entrance. LPA met with Residential Care Director, administrator was notified but was able to attend the visit.

The purpose of this visit is to verify that Staff (S1) is not on the property. LPA provided a copy of Decision and Order to administrator via email on June 13th, 2025, as mailed copy was sent to main office. S1 Last day at the facility was on June 12th, 2025 Residential Care Director understands that S1 is excluded and not permitted to be on the grounds at any time.

An exit interview was conducted and a copy of this report provided for facility records
