Facility Evaluation Report

On July 16, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit. LPA met with Program Director Irma Torres.

LPA explained the reason for the visit was due to incidents that happened at the facility with R1 & R2, which are two separate incidents.

The Dept received a report from the facility reporting R1 had spit on individual. Administrator explained R1 had become upset and at one point spit on a staff member. LPA reviewed R1's IPP which states R1 has a 1:1 staff while at Day Program. LPA verified R1's 1:1 staff while at the facility. R1 has a current diagnosis of Dementia. LPA informed Administrator to keep reporting incidents and if behaviors increase they will have to reach out to R1's home & CVRC service coordinator.

LPA also follow-up on R2's incident that was previously reported. Report stated R2 had punched staff member. Program Director stated this was an isolated incident and R2 has never hit anyone previously. Program Director stated R2 lives at home and has informed family of the incident and to follow-up with doctor if needed.

LPA did not observe any deficiencies during today's visit.
