Facility Evaluation Report

Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Irma Torres, Program Director.

Facility is licensed for 60 clients and has a current census of 44. Facility does not keep or give medications to clients. Water temperature was checked in the bathrooms which read at 114.8 degrees Fahrenheit. Fire Extinguisher was serviced 9/24/2025 and is within the safety regulation period. Smoke and carbon monoxide detectors were tested and in working order.

Facility is in the process of updating the Administrator. Staff files were reviewed, are complete, and current. Client files were reviewed. R1, R2, & R3 do not have current IPP’s. Facility provided a log for disaster drills that are conducted monthly. First aid kit on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA inspected the interior of the facility. Client’s bring their own lunches or buy lunch.

Deficiencies observed were cited during today's inspection per California Code of Regulations, Title 22.

LPA's requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, and LIC 610-E the Emergency Disaster Plan. Listed documents shall be sent to Licensing by 4/3/2026.
