Facility Evaluation Report
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility staff George Reither, and Administrator Kristian Torress by phone, and explained the purpose of today's visit.
LPA Hurt is following up on an incident reported involving a scratch Resident 1's shin. Administrator stated Resident 1 has been scratching the area on their shin and their nails being squared possibly caused the area to be more irritated. Administrator stated they will provide the medical discharge paperwork for Resident 1 by end of the day. LPA Hurt observed the facility is clean and did not see any type of issues that would cause Resident 1's skin scratch.
