The inspector’s account

Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced on 09/30/2025 to complete the annual inspection. LPA knocked on the door several times with no response. LPA Hurt spoke over the phone with facility administrator Gabriel Torres. Gabriel stated no one is at the facility and he is not currently in the area and all the residents are at the day program. LPA arranged with Administrator to complete the visit on another date.
