Facility Evaluation Report
At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit to follow up on some concerns reported by the Redwood Coast Regional Center. LPA met with caregiver Rey Mamuyac and toured the facility. The following items were inspected for compliance:
Client bedroom door locks. LPA observed all client bedroom doors had functional locks installed. Clients have their own key and staff have a duplicate incase of emergency.
First Aid Kit. LPA inspected several first aid kits in the facility. LPA recommended a higher quality first aid kit that contains all required items in one spot, instead of piecing together several.
Client Restroom. LPA discussed the reasons why staff should not utilize client restroom and to use the main common bathroom.
Menus. LPA observed current menus posted in the kitchen.
Medication Administration Record, MAR. LPA reviewed facilities MAR and observed all clients medication that was due at the time was signed off as given for the day.
Emergency Supplies. LPA inspected facilities emergency supplies. LPA observed there was sufficient food and water to ensure self sufficiency for at least 72 hours. LPA recommended an additional 10 gallons of emergency water to ensure all individuals, including staff, have sufficient water for drinking and hygiene needs.
Disaster Drill Log. LPA reviewed the facilities log for Fire and Disaster drills and observed the last drill conducted was November 2025. The facility plan states they conduct drills monthly. LPA requested facility complete a drill and submit self certification of completion.
Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
