Facility Evaluation Report

At approximately 10:45AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to two special incident reports submitted by the facility. LPA met with Program Supervisor Isidro Chavez and PQI Director Kristin Bogner and reviewed records. On 12/01/2025, staff discovered, during a change of shift medication count, a PM medication was administered in the morning. All notifications were made and staff was retrained. On 12/27/2025, after administering the morning medications, staff discovered they only gave one pill instead of two. All notifications were made. Staff responsible will be undergoing additional medication training.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
