Facility Evaluation Report

At approximately 8:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an unusual incident report submitted by the facility. LPA met with interim supervisor Laura Nava Lopez and reviewed records. The incident was in regards to a Client, C1, having two falls over the course of two days and being sent to the emergency room each time. C1 was admitted to the facility with a walker to aid in mobility. Based on admission documents, including the physician report, C1 was listed as being ambulatory. This facility has a fire clearance for 8 ambulatory clients. LPA discussed with supervisor the definition of ambulatory status in regards to the fire clearance. Supervisor will discuss with their team the possibility of submitting a request for Non-ambulatory status. LPA provided instructions for the submission of the request.
