This is an official report of an unannounced visit/investigation of a complaint received in our office on

10/18/2022 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20221018103021

At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct an investigation into the above allegation. LPA met with House Supervisor Barbara Driver, interviewed staff and Clients. Based on interviews conducted, LPA was not able to determine that staff were yelling at Clients. There are times when a staff member raises their voice to be heard from another room, but there were no witnesses to support the idea that a staff was directing their raised voice towards a resident in a hostile manner.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.
