Facility Evaluation Report

At approximately 11:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit regarding an Unusual Incident Report submitted by the facility. LPA met with Administrator Jotham Gikuhi. The facility reported there were documentation concerns with their Medication Administration Record, MAR. LPA spoke with Administrator and was informed the medications were given, but staff did not sign the form. Administrator has scheduled training for all staff to review medication documentation procedures and will review the MAR every week for accuracy.
