At approximately 10:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit to gather documents regarding former employees. LPA met with Executive Director Zenia Shah and reviewed records. LPA received copies of documents.

LPA will need to return on another day to conduct additional interviews.
