Facility Evaluation Report

At approximately 11:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to two (2) SOC 341 forms submitted by the facility. LPA met with Executive Director Zenia Shah and requested documents. The SOC 341 forms were in regards to a former staff, S1, having a verbal altercation with two different residents on two separate days. S1 was placed on leave and later terminated as a result of the incidents. LPA requested staff witness statements and a copy of the internal investigation.
