Facility Evaluation Report

Licensing Program Manager (LPM) Mota conducted an unannounced case management inspection and met with Zenia Shah, Administrator. The purpose of this case management inspection is to obtain additional information regarding 5 incidents self reported to Community Care Licensing (CCL) outlined below:

2/17/25 - Staff (S1) refused to assist in repositioning resident (R1).

2/26/25 - Staff (S2) spoke loudly to resident (R2).

3/12/25 - Staff (S3) failed to provide care to resident (R3)

4/20/25 - Staff (S4) spoke inappropriately to resident (R4).

4/15/25 - Staff (S5) was rough to resident (R5) while providing care.

An internal investigation was conducted and Staff (S1,S4 S5) were terminated. All incidents were property reported to required parties and the Ukiah Police Department.

LPM obtained additional documents and conducted interviews.

No deficiencies cited during today's inspection.
