Facility Evaluation Report

At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit. LPA met with new Executive Director Zenia Shah. LPA informed the Director of the previous complaint visit and the request from LPA that all future responses to the Resident council be sent to LPA in addition to the Council. LPA discussed areas of concern and possible solutions. LPA requested the documentation that would appoint her as the new Director.
