Facility Evaluation Report

At approximately 11:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an incident report submitted to CCL on 12/28/2022. LPA met with Executive Director Jeanette Kinney and reviewed records. Resident 1 had an un-witnessed fall in their bedroom and informed staff later of the incident. Resident complained of head pain. Facility staff contacted emergency personnel and resident was taken to the hospital. Resident returned a short time later with no new instructions.
