The inspector’s account

At approximately 8:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an unusual incident report submitted by the facility. LPA met with House Manager Alma Ayala and reviewed records. On 09/02/2025, staff was returning to the living room from the laundry room to check on Client, C1, when they observed the front door open and C1 walking up the ramp outside. C1 was previously sitting in their wheel chair in the living room. The report stated C1 managed to maneuver out the door and tipped their wheel chair over onto the lawn. Staff conducted an assessment for injury and found none. C1 was taken to the emergency room and returned the same day with no injuries. The incident report states the front door alarm was not turned on, allowing the door to be opened without staff knowledge.

LPA reviewed records and observed C1 has a history of attempted elopements and is to be with staff escort when leaving the facility.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
