Facility Evaluation Report
At approximately 8:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to an Unusual Incident report submitted by the facility. LPA met with House manager Alma Ayala and reviewed records. The incident occurred on 10/30/2025 when a client, C1, was observed to have a swollen finger. Prior to observing the swollen finger, on 10/07/2025, staff observed swelling on C1's left arm. C1 was taken to their physician for assessment. No cause was found and C1 returned to normal activities. On 10/29/2025, staff transported C1 to the physician again for a swollen left finger. An X-ray was performed and a fracture was discovered. Staff transported C1 to the emergency room on 10/30/2025, per physician instruction, to have the finger addressed.
LPA reviewed records and observed C1 has a history of seizures. Staff reported they did not witness any events that could have caused the injury and ensured medical attention was attained when the injury was observed. LPA obtained copies of documents.
