HOLY SPIRIT RESIDENTIAL CARE HOME INC. II

At approximately 08:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived to this facility, unannounced, to conduct an investigation into the above allegation. LPA met with Administrator Perla Gonzalez and toured the facility. During the tour, LPA observed the main entry, inside door handle was removed. Upon further investigation, a residents bedroom exit door was in the same condition. An immediate civil penalty is being assessed in the amount of $1000.00, for this repeated violation in a 12 month period. LPA discussed with Administrator the importance of keeping the door handles in place and finding other ways to redirect exit seeking behavior.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

This report was reviewed with Administrator and Appeal rights were given.
