Facility Evaluation Report
On 06/16/2026, at approximately 2:55 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a case management - incident visit and met with Sheri Lott, Administrator.
LPA is conducting a case management visit to obtain more information regarding an Incident Report received by the Department on 06/08/2026 for concerns reported by Resident 1 (R1) that Resident 2 (R2) entered R1's bedroom and engaged in sexual behavior. LPA made observations, obtained documents, and conducted interviews.
No deficiencies were cited during todays visit.
Exit interview was conducted with Sheri Lott, Administrator, whose signature on form confirms receipt of documents.
