Facility Evaluation Report

A non-compliance conference was conducted today in the Santa Rosa Regional Office. Present in the meeting: Regional Manager, Carla Nuti-Martinez, Licensing Program Manager, Bethany Moellers, Licensing Program Analysts, Marisol Cuadra and Shannan Hansen via Teams, Facility Licensee, Matt Omelagah, Facility Administrator, Sheri Lott, Golden Gate Regional Center Representatives, Jennifer Smith and Lindsey Reed. The purpose of today's meeting is to discuss lack of care/supervision resulting in a client falling over 2 nd story balcony sustaining multiple injuries and ongoing compliance concerns not limited to below.

On 2/5/2026 an immediate civil penalty in the amount of $500 was issued, licensee was informed that additional civil penalties are under review by the Department per Health and Safety Code 1569.49 (f). This is related to the incident noted above.

Licensee was informed of technical support program and agrees to engage in services. Licensee will provide copies of their internal investigation not later than COB 3/20/2026
