Staff is denying a client from using the restroom
On 02/05/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced visit for the purpose of delivering complaint findings. LPA arrived and met with Administrator, Sheri Lott. During the course of the investigation, LPA reviewed records, conducted interviews, and made observations.
Compliant alleges, staff yelled at clients in care and staff is denying a client from using the restroom.
Based on department documentation and interviews, the following determination has been made. Interviews conducted with three out of five staff, revealed that they have heard staff yelling at clients. Three out of four staff interviewed revealed that staff will lock the bathroom door to prevent client (C1) from using the bathroom as C1 will get feces and blood over the bathroom.
Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Division 6, Chapter1 is being cited on the attached LIC 9099D. Appeal rights given.
