Facility Evaluation Report
Licensing Program Analyst (LPA) Loera was at facility conducting a complaint investigation and conducted a case management. LPA met with Paula Sifflet, Regional Manager. The purpose of this case management was address an issue found during conducting a complaint investigation.
While conducting investigation into allegations of complaint 21-AS-20251226091647 and 21-AS-20251231112418. LPA was informed through interviews; the food quality provided to clients is moldy and expired. LPA conducted an inspection of food quality and observed two bags of spoiled spinach with brown liquid inside and moldy bell peppers (Deficiency Cited).
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided.
