LPA Loera conducted an unannounced visit to follow up on an incident report that was sent to CCL on 04/07/2025.
LPA was informed when an incident happens, staff have a communication log for each client where staff document what happened so the next shift is aware. Staff are directed every day to look at the log and document. LPA was also informed when an incident occurs lead staff reports to Regional Manager and/or behavioral specialist.
