Facility Evaluation Report
Licensing Program Analysts (LPAs) Victoria Bertozzi and Helena Rummonds arrived unannounced to conduct an Annual Required inspection and met with Regional Manager, Paula Sifflet.
LPAs initiated a tour of the facility around 10:15am and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Client rooms were furnished per regulation. Water temperature in sinks accessible to clients measured at 102 degrees F which is not within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Additional cleaning supplies and disinfectants are stored in garage closet that is inaccessible to clients. Facility has at least two days of perishable and one week of non-perishable foods which was stored per regulation but not all were of quality. Medications were centrally stored and locked. Emergency food and water supplies are stored in the garage along with Personal Protective Equipment.
Fire extinguisher was last serviced June of 2022. Facility has Smoke and Carbon Monoxide detectors located throughout the facility that were tested and operational during visit. Most recent fire/disaster drill was conducted in May of 2022.
Five staff files and five client files were reviewed. Administrator Certificate for Administrator Sheri Lott 6029115735 expires 11/17/2023. Staff training was reviewed. Medications and medication records and client cash resources were also reviewed. LPA observed missed medications due to medication not being refilled.
