At approximately 12:50 PM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a case management-other inspection and was greeted by Assistant Director Sabrine Falla.
The purpose of this visit was to follow-up on a self-reported incident submitted by the facility on 9/4/2025. Client (C1) left the facility premises before being found and treated at a community hospital. LPA requested documents and conducted interviews.
No deficiencies cited. Exit interview conducted with Assistant Director, whose signature on form confirms receipt.
