The inspector’s account

At approximately 9:30 AM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Assistant Director Sabrine Falla. Director Ketura Topps was contacted via phone and gave permission for Assistant Director to sign and receive report. Facility is a Social Rehabilitation Facility (SRF) with an approved fire clearance and a capacity for sixteen (16) Ambulatory clients. Upon arrival, LPA was informed that there were thirteen (13) clients in care, and three (3) staff members on-site.

At approximately 10:00 AM, LPA initiated a tour of the facility with Assistant Director and observed the following: Facility is a one (1) story facility, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguishers were observed charged and last serviced 11/2024. Smoke and Carbon Monoxide detectors are serviced by a third-party vendor and were last serviced 4/2025. Water temperatures in client accessible sinks read between 114.2- and 117.1 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, and paper products available for clients. Clients' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulation. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two (2) days of perishable food and seven (7) days of non-perishable foods, as well as an emergency water supply. LPA observed a computer for client usage as well as two (2) phones for clients to make calls and there is internet available to clients in care. LPA observed multiple activity rooms where clients are able to play instruments, watch movies, create art, and socialize. There is a shaded seating area in the back lot for client use and the facility phone was observed functional during this visit.
