Facility Evaluation Report
At approximately 10:20AM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management - Licensee Initiated visit. The purpose of the visit is to confirm that the facility is no longer in operation.
On 04/01/2026, LPA received an email from Regional Licensing and Compliance Manager, Crystal Butler, stating that they were surrendering the license for Newport Institute – Bayview, Facility Number, 216804061. Per Regional Licensing and Compliance Manager, there are no staff members available in Marin County to meet with LPA for the closure visit. LPA was also informed that the original license was disposed of. Facility's Lease Termination document was provided to the Department on 04/07/2026.
LPA inspected the exterior of the building and found no evidence that would suggest that facility staff or clients were still on the premises. The closure of this facility has been finalized. Facility is closed as of today, with an effective date of 04/15/2026.
Regional Licensing and Compliance Manager to be informed that if they wish to open a facility, a new application will need to be submitted to the Department for review and approval.
LPA were unable to conduct an Exit interview or obtain a staff signature as there were no staff present during visit. LPA to provide a copy of report to Regional Licensing and Compliance Manager via email to obtain electronic signature for facility file.
