Facility Evaluation Report

At approximately 11:30AM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management - Deficiencies visit, and met with Administrator, Reggie Catiis, Program Supervisor, Ilse Resendiz, and Assistant Director, Erika De Vera.

During the course of Complaint Investigation, Control Number 21-AS-20260112152456, the following deficiencies were identified:

Facility did not report to Community Care Licensing (CCL) in an LIC624 or SOC341 regarding C1 and concerns of alleged bruising.

Facility did not follow C1's ISP dated 03/04/2024 . Review of C1's ISP stated that C1 should have a minimum of 45 minutes dedicated to getting them prepared for transportation. Per interviews conducted facility staff stated that when C1 is not wanting to get on the bus they are given some time and are told that their friends are on the bus or that their family is waiting for them. C1 is also given 5 -10 minutes and a countdown.

Facility did not ensure C1's personal rights. Facility narrative forms found in C1's file stated that facility staff picked C1 up by their waist and by their feet to place them on the transportation bus on 01/06/2026. Review of C1's nursing needs assessment dated 05/03/2021 stated that C1 is ambulatory and is able to walk.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Exit interview conducted. Copy of report, LIC-809D, Plan of Corrections, and Appeal Rights discussed and provided to Administrator. Signature on form confirms receipt of documents.
