The inspector’s account

09/18/2025, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. Facility is an adult day care program. LPA met with Program Supervisor, Jairo Guillen-Vasquez. Day Program has a fire clearance for 30 non ambulatory clients. Upon arrival 26 clients were attending day program and 20 staff were on sight. Facility is a one story building with a multipurpose area, three activity rooms, laundry area, kitchen, 2 bathrooms, a nurse's room and a staff office.

LPA and Program supervisor toured the building and grounds. The facility was found to be at a comfortable temperature.

LPA observed clients participating in activities during the tour. Facility has activities available to clients such as puzzles, games, art supplies, outings, and various other activities. Water temperature in sinks accessible to clients were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were last inspected December 2024. Facility has a hard wired fire alarm and sprinkler system that is directly connected to the local Fire Department. Facility's smoke detectors and sprinkler system were last tested 01/07/2025. Facilities last emergency drill was conducted 09/05/2025. Sharps, toxins, and chemicals were locked and secured in the kitchen and laundry room. Facility does not handle client cash resources. Facility maintains a Medication Authorization Record (MAR) of medications being administered which was observed to be appropriately maintained.

LPA conducted a review of 6 client records. All records had the required documentation.

LPA conducted review of 6 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file.
