Based on interviews and records reviewed, the allegation of Hospice Nurse was denied entry into the facility is Substantiated. The preponderance of evidence standard has been met and the above allegation is SUBSTANTIATED. Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Exit interview conducted and appeal rights were given. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties.
