Facility Evaluation Report

At approximately 8:45AM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management - Incident Visit and met with Team Leader, Zasha Daep. Assistant Program Director, Suhay Rivas arrived during visit at approximately 9:54AM. The purpose of the visit was to follow up on incident reports that were submitted to Community Care Licensing (CCL).

Review of Incident Report 1 and Incident Report 2 stated that on 09/19/2025 and 10/23/2025 Client 1 (C1) was administered their medications outside of their prescribed time by Staff Member 1 (S1) and Staff Member 2 (S2) (deficiency cited, regulation 80075(b)).

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Exit interview conducted. Copy of report, LIC809D (Deficiency Page), Plan of Corrections, and Appeal Rights discussed and provided to Team Leader and Assistant Program Director. Signature on form confirms receipt of documents.
