At approximately 12:00PM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management - Other visit and met with Executive Director, Shannon Brown, and Charge Nurse, Melanie Fenn. The purpose of the visit is to follow up on a plan of correction cited on 12/29/2022 and review expectations for annual visits.

Updating Staff and Resident Files as needed
Exit interview conducted. Copy of report discussed and provided to Administrator. Signature on form confirms receipt of documents.
