Facility Evaluation Report
06/25/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan and infection control plan. Facility is an adult day care program. LPA met with Program Manager, Yasmin Boroumand . Day Program has a fire clearance for 49 clients.
LPA and Administrator toured the building and grounds. The facility was found to be at a comfortable temperature. There are 49 clients that are enrolled in day program. Today there are 26 clients in attendance.
LPA observed clients participating in activities during the tour. LPA toured multiple activity rooms, kitchen, offices, and bathrooms. Water temperature in sinks accessible to clients were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were last inspected March 2026. Facility has a pull station that is tested by the Fire Marshall's twice a year. Toxins and cleaning supplies are stored in locked supply room closet and in locked cabinet. LPA was informed that clients bring their own lunches to program. Day program has a refrigerator along with other kitchen appliances for clients to use. One client takes medication which is locked in the program managers desk.
Facilities last fire drill was conducted on April 29, 2026.
LPA conducted a review of five client records. All records had the required documentation.
