Facility Evaluation Report

Licensing Program Analyst (LPA) Hansen arrived unnanounced at facility to conduct a Case Management - Legal/Non-compliance Inspection. LPA knocked, and rang belll, then LPA called Administrator Willie Hogan who arrived 10 minutes later and was able to conduct case management. LPA conducted a walk-through of the facility, that was found at a comfortable temperature with all exits free from obstruction. This facility was placed on non-compliance on September 6th, 2024 for a two-year term.

LPA went over compliance plan and reminded facility of the below agreement of 9/6/2024 between Community Care Licensing (CCL) and Facility, Sycamore Home.

A copy was provided to the administrator. No citations issued during todays visit.
