Facility Evaluation Report
Licensing Program Analyst (LPA) Stevenson arrived unannounced to conduct a Case Management visit and met with administrator Jasmin Gongora. The purpose of today's visit was to ensure that the delayed egress was properly functioning and to follow up on an outside report of the delayed egress malfunctioning and reporting requirements not being met per Title 22 Regulations to Community Care Licensing (CCL).
On 10/3/25, it was reported to CCL from an outside party that on 9/12/25 client (C1), who was in crisis, was able to leave the home "without any issues due to the delayed egress still being down (work order submitted 7/14/25)". Although C1 was in line of site of staff, C1 sat in the street where staff, were then required to redirect traffic around C1. C1 did return to the facility at approximately 10:00 pm. At approximately 11:05 pm on this same date, C1 once again was able to "run out of the house successfully without any delay due to the delayed egress not working" and again sat in the roadway blocking traffic with staff monitoring C1.
CCL was not provided an Unusual Incident Report regarding the delayed egress not functioning or the incident involving C1 per regulation 80061 – Reporting Requirements.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Appeal rights given and discussed with Administrator. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
