Facility Evaluation Report
04/02/2026, Licensing Program Analyst (LPA) Loera arrived unannounced to conduct a case management inspection and met with Administrator, Jasmin Gongora. The purpose of the case management visit was to follow up on two self reported incidents submitted to Community Care Licensing (CCL) on 04/01/2026.
Incident report #1: On 04/01/2026, it was reported to CCL that on 03/30/2026 client (C1) had been sexually assaulted by client (C2). Program Administrator arrived at the house around 9AM and staff let Administrator know that C1 had began yelling and crying stating they were in pain. Administrator directed staff to give C1 a PRN for pain to see if it would help. C1 continued yelling and Administrator called C1 into their office and asked C1 if they could point to the area it hurts. C1 then pointed to their anus and Administrator asked C1 why was it hurting there and C1 stated they already told them they do not want it in there.
Incident report #2: CCL received an incident report on 04/01/2026, stating on 03/30/2026 C2 was arrested and booked on pending charges.
Per document review and conversation with Administrator, facility staff contacted the police, C1s and C2s behavioral team and social workers.
LPA reviewed documents and made observations.
Exit interview conducted with Administrator and copy of report was provided.
