Facility Evaluation Report
04/07/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently 3 clients in care. Facility approved/cleared for 6 ambulatory only. LPA was greeted by House Manager. Licensee/Administrator, Judith Anglade was available by phone. Upon arrival two clients were present and one client was attending day program.
LPA conducted a review of 3 staff records/training. Upon a review of staff records, LPA found all staff to have required training as well as current 1st Aid & CPR certification on file. LPA conducted review of three client records. LPA observed three out of three clients to be missing a medical assessment (LIC602) (Deficiency Cited). Facilities last fire drill was conducted 03/02/2026.
LPA and House Manager toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. LPA observed a single sized mattress with pillows and sheets located in the garage under a large wooden shelf (Deficiency Cited). All rooms were furnished per regulation. Extra hygiene products and linens were available. Water temperature in sinks accessible to clients in care were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were found to be recently purchased and fully charged. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Toxins and sharps were found to be under the kitchen sink and were secured. Medications were found to be centrally stored. LPA conducted spot medication count and found expiration dates to not be filled in, date filled for medications were inaccurate, and date started were not filled in (Deficiency Cited).
