Facility Evaluation Report

Licensing Program Analyst (LPA) Loera arrived unannounced to conduct a Case Management - other and met with Administrator, Rachel Hylton and Administrator Nancy Morquecho. The purpose of the visit was to provide information regarding regulations 87615- Prohibited Health Conditions, 87609- Allowable Health Conditions and the Use of Home Health Agencies, and 87616- Exceptions for Health Conditions.

LPA discussed with administrators the above regulations as facility is not allowed to retain or accept a resident with a stage 3,4, and/or unstageable wound as per regulation 87615. Facility is required to have all home health records on file in the facility as per regulation 87609.

LPA provided administrators with copies of regulations 87615, 87609, and 87616.

No deficiencies cited during todays visit.
