The inspector’s account

04/21/2026; LPA Loera arrived unannounced to conduct a Case Management - Incident. Upon arrival no staff or clients were present. LPA made phone calls and got a hold of Manager, Rodric Robinson. Staff arrived at approximately 2:00pm. Administrator was unable to come during visit. The purpose of the visit was to follow up on self-reported incident that were submitted to Community Care Licensing (CCL).

CCL received an incident report on 04/07/2026 stating on 04/04/2026, in the morning it was noted that client (C1) missed their afternoon medication of two doses. C1 missed medication doses for both Saturday and Sunday. The situation was reported and an investigation was performed on Monday morning and the findings showed on the weekend, afternoon does of medication for C1 were sent to C1s day program in error.

Per conversation with manager, C1s day program receives C1s medication for the month. The staff member in charge of sending the medication to C1s day program was supposed to send medication for only Monday through Friday but ended up sending two additional days of medication resulting in C1 missing their medication on the weekend (4/4/2026 & 4/5/2026).

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Exit interview conducted. Copy of report, LIC809D, LIC811, and Appeal Rights provided to staff member.
