Facility Evaluation Report

07/02/2026; Licensing Program Analyst (LPA) Loera arrived unannounced to conduct a Case Management - Incident visit. LPA met with Administrator, Mayeen Galang. The purpose of the visit was to follow up on self-reported incident that was submitted to Community Care Licensing (CCL).

CCL received an incident report on 06/03/2026 stating on 06/02/2026, at approximately 11:30 AM resident (R1) fell and struck their head when being picked up for a dentist appointment. R1 was transferred to the Novato Community Hospital where a CT scan of the head was performed and a subdural hematoma was noted.

Per conversation with Administrator, R1 had the fall at day program when getting into a vehicle and lost their balance. R1 received medical treatment and returned to the facility on 06/04/2026. R1 received home health service visits for a week and was observed to be doing well to which home health services were stopped.

LPA obtained copies of medical documents.

Exit interview was conducted with Administrator. Copy of report was provided.
