Facility Evaluation Report

06/13/2025, Licensing Program Analyst (LPA) Loera arrived unannounced to continue an Annual Required inspection that was initiated on 05/14/2025, and was greeted by administrator, Maria Lepe.

LPA conducted a review of three client records. All records had the required documentation.

LPA conducted review of three staff records/training. Upon a review of records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file.

LPA conducted spot medication count and found medication to be properly recorded on the Centrally Stored Medication log. LPA reviewed reviewed client cash resources.

Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 07/13/2025:

LIC400 - Affidavit Regarding Client/Resident Cash Resources

Exit interview conducted and copy of report was provided to Administrator.
