Facility Evaluation Report

Licensing Program Analyst (LPA) Loera conducted an unannounced case management inspection and met with Krysteena Brice, Manager. The purpose of this case management inspection was to get statements for both incidents submitted to Community Care Licensing (CCL) on 01/30/2025 and 03/12/2025.

LPA requested S1 if they could complete an LIC855 Declaration, documenting their statements on both incidents and any additional information if applicable.

S2 was not present at facility during visit so Facility Manager will ask S2 and forward to CCL if S2 agrees to sign and date their statement.

Exit interview conducted with Manager and a copy of this report given. No deficiencies cited.
