Facility Evaluation Report
At approximately 12:45 PM, Licensing Program Analyst (LPA) Hansen arrived to conduct an unannounced annual inspection of this Social Rehabilitation Facility and was welcomed by staff Josclyn who contacted Program Director, Jade Dolcini that arrived shortly after. Facility is a two-story residence with 5 bedrooms, 3 bedrooms on the main floor and 2 on the second story. Fire clearance has been approved for 6 ambulatory clients by the Novato Fire Protection District.
At approximately 1:00pm a tour of building & grounds including facility’s bedrooms, kitchen area, dining room, laundry room, living room, group room, recreation room, and staff office. A remodel of garage was conducted and approved 6/9/2025 now containing 4 clinical offices, a nurses office/medications, gym, and additional bathroom. The facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Hot water temperature measured between 78 degrees F and 121.6 not within required range of 105 degrees F and 120 degrees F in 3 out of 5 client’s bathroom faucets as required by Title 22 Regulations (see LIC9012 TA). Toxins were secure and inaccessible to clients. There was a sufficient supply of hygiene products, paper products, and linens available for client use. All rooms were equipped with lighting, night stand, and drawers. All rooms were in good repair.
LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner being labeled and dated. Facility sharps such as knives were stored and locked in the facility's pantry room and locked kitchen drawer.
At approximately 2:00 pm, LPA began a review of five (5) client records but was not able to complete and will return at a later date to finish inspection of staff records and medication review.
Exit interview conducted. Copy of report discussed and provided to Program Director. Signature on form confirms receipt of documents.
