Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility Program Director, Beth Chudy and explained the purpose of today's visit.

LPA reviewed an Incident report regarding a vehicle accident that took place on 05/07/2026. Program Director arrived on the scene within minutes of the incident. Staff 1 immediately assessed all clients for injuries. No injuries were reported. Program director did notify responsible parties of residents involved and notified Licensing.

Technical assistance was provided, Exit interview conducted with facility Program Director, Beth Chudy, and a copy of this report provided.
