Facility Evaluation Report

Licensing Program Analyst (LPA) Angela Barutyan conducted a case management - other visit with the purpose of conducting a final walk-through of the facility prior to closure. LPA arrived at the facility at 10:40AM and observed no one present. LPA attempted to contact the Licensee via telephone and was unable to reach Licensee. At 10:42AM, LPA contacted the Licensee’s secondary contact, Erwin Banola, and explained the reason for the visit. No one was able to meet with LPA for the visit.

On 06/23/2025, the Licensee visited the Woodland Hills North Regional Office and made a verbal request to close the facility to the on-duty LPA.

During today’s visit, LPA was unable to enter the property. LPA observed a for-sale sign in the front yard, a padlock on the gate, and a lockbox. LPA did not observe any lights on inside and property appeared vacant. No signs of residents or others living in the property were observed. LPA could not find the property listed for sale online. At 11:09AM, LPA contacted the real estate agent listed on the for-sale sign and agent stated that the property will be listed online today, 07/10/2025. At 01:12PM, Mr. Banola confirmed with Licensee and informed LPA that the last resident moved out on 05/12/2025 to another licensed facility.

Based on the LPA's observations during today's visit, the LPA concluded that all operation of the Residential Care Facility for the Elderly has ceased.

The Licensee surrendered the License to the Woodland Hills North Regional Office on 06/30/2025.

Licensee was unable to be present for the visit. A copy of the report will be mailed to the Licensee for a signature. Exit interview conducted telephonically. A copy of the report was emailed to Mr. Banola.
